Information on this Site—including quiz results, descriptions of symptoms, explanations of services, or marketing content—is not medical advice, and must not be interpreted as:

• treatment plan
• clinical instructions
• prescriptive health advice

Massage therapists in Alberta are not regulated health professionals, and all services are provided within the standard scope of massage therapy under Alberta law.

Always consult a regulated health professional for medical concerns.

3. No Therapist–Client Relationship Is Created

Submitting your information or interacting with the Site does not create a client–therapist relationship with any therapist at Maximum Health Wellness Centre Inc.

A client relationship is only formed after:

• You book an appointment through JaneApp,
• You complete an official intake form, and
• You attend your scheduled session.

4. Accuracy of Information

You agree that:

• All information you submit through the Site (including quiz responses, contact details, and any health-related descriptions) is true and accurate.
• You will not knowingly provide false or misleading information.

We are not responsible for problems resulting from inaccurate, incomplete, or misleading information submitted by users.

5. Acceptable Use

You agree not to:

• use the Site for unlawful purposes
• attempt to gain unauthorized access to the Site or its servers
• submit malicious content, spam, or harmful code
• interfere with the Site’s functionality
• reproduce or copy content without permission
• use the Site to harass, threaten, or harm others

We may terminate your access to the Site at any time for violating these Terms.

6. Use of Forms, Quizzes & Submitted Information

When you submit information through the Site, you acknowledge and agree that:

• The quiz is for general informational matching only, not diagnosis or treatment.
• Personal health information (PHI) entered into the quiz is stored in an access-controlled encrypted database within our controlled infrastructure, physically separated from any contact information you provide. See the Privacy Policy Section 5 for details.
• Only your contact information and advertising identifiers (name, email, phone, IP address, gclid, fbclid, etc.) are exported for follow-up and advertising analytics.
• We may use your information to:
• contact you
• respond to your inquiry
• send optional marketing communications (with consent)
• perform offline conversion tracking
• improve advertising performance

All handling of your personal information is governed by our Privacy Policy, which forms part of these Terms.

7. Third-Party Services

The Site may include integrations or data collection by:

• Google Analytics
• Google Ads
• Google Tag Manager
• Microsoft Clarity
• Meta/Facebook Pixel
• JaneApp booking platform
• Email marketing or CRM tools (if implemented)

These third parties have their own terms and privacy policies, and may store information outside Alberta or Canada.

We are not responsible for:

• the content, policies, or practices of third-party services
• downtime or errors resulting from those providers
• any decisions you make based on third-party content

When you click to book, you are redirected to JaneApp, which is governed by its own separate terms.

8. Intellectual Property

All content on the Site—including text, images, graphics, logos, layout, and marketing copy—is the property of Maximum Health Wellness Centre Inc., unless otherwise noted.

You may not:

• reproduce
• distribute
• modify
• translate
• republish any content without our express written permission.

9. No Guarantee of Results

Massage therapy outcomes vary from person to person. We do not guarantee:

• pain relief
• specific functional changes
• measurable results
• suitability of any particular service

Marketing examples or testimonials shown on the Site are illustrative only.

10. Disclaimer of Warranties

The Site is provided “as is” and “as available” without warranties of any kind, whether express or implied.

We do not guarantee that:

• the Site will be uninterrupted or error-free
• defects will be corrected
• tracking tools will function as intended
• the information provided is up-to-date or complete

Use of the Site is at your own risk.

To the fullest extent permitted by Alberta law, Maximum Health Wellness Centre Inc. is not liable for any damages arising from:

Your sole remedy for dissatisfaction with the Site is to discontinue its use.
